Referring a Patient

At Cardinal Hospice of South Texas, we accept referrals from healthcare professionals, family members, or loved ones at any stage of a patient’s illness. However, we strongly encourage early referrals, as research shows that patients benefit greatly when hospice services are introduced sooner. Early involvement allows us to build meaningful relationships with both the patient and their loved ones, understand their unique needs, and coordinate care that enhances quality of life.
Determining when a patient is appropriate for hospice can be challenging. Below are some general indicators that may suggest a hospice referral is appropriate:
• Persistent or chronic symptoms impacting quality of life
• Ongoing decline in physical function
• Unintentional weight loss of more than 10% in the last six months
• Multiple chronic health conditions
• Need for assistance with most daily activities
• Frequent infections
• Repeated hospitalizations or emergency room visits
• Wounds that are not healing
Our team is here to support you in evaluating hospice eligibility or guiding sensitive conversations with patients and families.
Once a referral is made, one of our experienced Registered Nurses will visit the patient to assess their needs and develop a personalized care plan. Every referred patient also gains access to our 24/7 support line for guidance and reassurance at any hour.
Our goal at Cardinal is to review referrals promptly, gather all necessary information, and schedule a visit within 24 hours or less—because no one should be left waiting for care or living in fear.
